It is the responsibility of the client to maintain correct personal information including address, telephone and e-mail address and other important information.

 

Please be on time for your appointment.  It is important to us to have scheduled time available for our clients.  If a client is more than 15 minutes late, we reserve the right to reschedule the appointment to another day and/or time to enable us to provide the best service to all of our clients.

 

If a client needs to cancel or reschedule an appointment, the client must give our office 24 hour notice, otherwise the client will be considered a no show appointment.

 

A client's failure to notify our office of a need to cancel or reschedule with at least 24 hour notice will result in a $25.00 no show/cancellation fee.  Pre paid treatments may result in forfeit of one treatment.
